Applications

Prospective students can obtain program application packets by clicking the links below and then either downloading or printing the related files.  The files are in PDF format.  Adobe Acrobat Reader is required to view these files and is available free by clicking on the link.  All completed applications must be mailed to the PT Program.

	Application Packet - Graduate Program in Physical Therapy 2009 Application Deadline for receipt of the application by the Graduate Physical Therapy Program is March 1, 2009.  If you are applying to the graduate program, you must also apply for admission to the Graduate School at ASU.
	Application Packet - The application for the Physical Therapist Assistant Program for students wishing to be admitted in 2009 will be made available in December of 2008.  Contact the PTA Program for more information. Deadline for receipt of the PTA application by the Program is April 1, 2009.
