Prospective students can obtain program application packets by clicking the links below and then either downloading or printing the related files.  The files are in PDF format.  Adobe Acrobat Reader is required to view these files and is available free by clicking on the link.  All completed applications must be mailed to the PT Program.

	Application Packet - Graduate Program in Physical Therapy 2007-2008 Application Deadline for receipt of the MPT application by the Program is March 1, 2008.  If you are applying to the MPT program, you must also apply for admission to the Graduate School at ASU. 	Application Packet - Physical Therapist Assistant Program 2007-2008 Deadline for receipt of the PTA application by the Program is April 1, 2008.
